The Standing Committee on Vaccination (Stiko) in Germany has issued a new recommendation to vaccinate children between the ages of 12 and 14 against meningococcal bacteria. This advisory aims to protect adolescents, a group identified as having a higher risk for severe meningococcal disease, and to help reduce the overall spread of these bacteria within the population.
Meningococci are bacteria transmitted through droplets, such as those released when coughing or sneezing. These pathogens can cause serious illnesses, including meningitis (inflammation of the membranes surrounding the brain and spinal cord) and sepsis (blood poisoning). Although occurrences in Germany are currently rare, infections are often severe and can lead to significant health consequences or even be fatal. Data indicate that the mortality rate ranges from 7 to 15 percent, and many survivors suffer from lasting health problems.
Adolescents at Increased RiskEpidemiological research shows that teenagers aged 15 to 19 are particularly vulnerable to invasive meningococcal infections, especially those caused by serogroups A, C, W, and Y. To establish protection before reaching this high-risk age group, Stiko advises that immunization be administered between ages 12 and 14. The recommended vaccine covers all four of these serogroups in a single combination dose, streamlining the vaccination process and increasing convenience for families and healthcare providers.
This vaccination can be integrated into the routine adolescent health check-up known as J1, which is typically conducted during early teenage years. Individuals who are older and did not receive the vaccine as recommended are encouraged to undergo catch-up immunization until their 25th birthday.
Update to Vaccination Protocols for ChildrenThe latest recommendations have also led to adjustments regarding meningococcal vaccinations for younger children. Previously, vaccination against meningococcal serogroup C was part of the standard immunization schedule for children starting at two years old, with a catch-up vaccination advised up to age 18. However, with the incidence of serogroup C infections declining steadily over recent years, this vaccination is no longer routinely advised for young children. According to the Robert Koch Institute (RKI), cases involving this serogroup have become exceedingly rare, diminishing the overall benefit of administering the vaccine to this age group.
Nonetheless, infants remain susceptible to infections from meningococcal serogroup B. As a result, the recommendation to vaccinate infants against serogroup B, starting from two months of age, remains in place. This vaccine is delivered in a series of doses at 2, 4, and 12 months, with catch-up doses recommended for children up to five years old. The ongoing prevalence of serogroup B in young children underpins the continued emphasis on early immunization.
Implementation and CoverageHealthcare providers are expected to begin offering the combined vaccine for serogroups A, C, W, and Y in the coming months. However, before widespread administration can occur, arrangements regarding coverage by health insurance providers must be finalized. The process involves coordination between healthcare authorities and insurers to ensure accessibility and reimbursement for eligible patients.
The updated vaccination recommendations are part of an ongoing initiative to adapt public health strategies in response to shifting epidemiological trends. The goal is to provide optimal protection to those most at risk while efficiently allocating healthcare resources. By updating immunization schedules based on current data, health authorities aim to minimize the impact of serious infectious diseases among children and adolescents in Germany.
